There are several ways to request your medical records:
Through MyChart
MyChart is a secure and free online tool that connects patients electronically to portions of their medical records. This health information granted to registered users. All users need is an internet connection and computer, smartphone, or tablet access. This service lets you view and track your medications, allergies, immunizations, and medical history. You can also request your full medical records history through MyChart. We will provide you with the information on how to acquire your medical records or have them sent to your provider. In-Person
Drop in to Kintegra Customer Support at 613 E. Franklin Blvd., Gastonia. You’ll be asked to sign our Release of Information form, and our staff can then provide you a copy of your records, or forward them on to your designated provider upon your instruction.
By Phone
Call our Medical Records Department at 704 874 1983. Our staff will provide you with the information on how to acquire your medical records or have them sent to your provider.
At a Kintegra Practice.
Drop into your Kintegra Practice. You’ll be asked to fill out a ‘Release of Information’ form. Be sure to bring a photo ID. Our staff will process the request to have your records available for pick-up or sent to you or your provider.
