Kintegra Health has added Community Resource Advocates (CRA) to our Family Medicine practices. The CRA staff is dedicated to providing free enrollment assistance, and referral support to all patients, staff and community partners we serve. Through free CRA appointments, individuals/families will be empowered to make informed decisions about their healthcare; and to become agents of change concerning access to affordable healthcare and other equitable resources. Please see below for an overview of our services.

• Connection to food through enrollment in Super SNAP and/or food pantry referrals.
• Connection to healthcare through enrollment in ACA insurance, Medicare Part D, or Kintegra’s uninsured patient program.
• Assistance with enrollment, exemptions and understanding your individual health insurance options
• Assistance with enrollment in Medicaid and other Social Services programs offered via the E-PASS portal.
• Connection to internal Kintegra programs such as Medication Assisted Treatment for Opioid Use Disorder, behavioral health counseling, HIV/AIDS treatment and case management, Hep C services, prescription assistance, and endocrinology services.
• Provide education and assistance with disability applications through the SOAR program.

SHIIP Services 

CRA staff members are certified SHIIP (Seniors’ Heath Insurance Information Program) counselors who enroll and counsel Medicare beneficiaries and caregivers about Medicare, Part-D plan options.

SuperSNAP Assistance 

Patients of Kintegra Health, currently receiving EBT/SNAP benefits, and/or have been affected by COVID-19, are eligible to receive SuperSNAP. SuperSNAP – is a benefits program in partnership with Food Lion stores, which provide an additional $40 per month, on your Food Lion savings card, toward the purchase of fresh fruits and vegetables. To enroll in English call 704.874.0571 Para Espanol 704.853.4631.
