Value Based Care and ACO Partnership

As Kintegra, like many health care organizations continues its strategy to move towards value-based healthcare, value-based care collaboration is key to ensuring the delivery of quality care and improved patient health outcomes.

An ACO is a group of doctors, hospitals, and other health care providers who come together voluntarily to give coordinated high quality care to their Medicare patients. The goal of coordinated care is to ensure that patients, especially the chronically ill, get the right care at the right time, while avoiding unnecessary duplication of services and preventing medical errors.

Kintegra Health partners with CHESS Health Solutions to provide care which results in better outcomes for individuals and patient populations. This value-based care collaboration works to understand the needs of Kintegra’s patient population and provides innovative high-touch services that address the needs of the sickest and most impacted individuals, while aligning with clinical and financial goals. This partnership aims to improve how care is delivered, lowers the cost of care, and, most important, achieves better patient outcomes.

Services provided include:

Pharmacy Support: (medication reviews, medication assistance, patient education, disease management, medication adherence and medication utilization)

Care Coordination: (Transition of Care, Emergency Department outreach and follow-up, Care Gap scheduling and closure, social determinants of health assessment and referrals, and annual wellness visit scheduling.

Social Work: (Addressing  social determinants of health (SDOH) and resource procurement, Advance Directives, Home Life reviews, safety concerns/abuse and neglect concerns, substance abuse referrals, assistance with placement for (SNF, ALF, etc. ), referrals to supportive counseling & behavioral health resources.

CHESS works closely with the Centers for Medicare & Medicaid Services (CMS) and manages several Accountable Care Organizations (ACOs), and the MSSP Shared Savings Program ACO models. (CHESS MSSP, LLC and CHESS Value, LLC).

 

Our Clinically Integrated Network (CIN) partnership with Aledade is focused on  supporting Kintegra with data analytics and technology, quality improvement guidance, patient education resources and provider support.

 

CCPN is a clinically-integrated network (CIN) of independent primary care physicians dedicated to improving quality, proving value to payers and patients, and keeping doctors and clinicians in control of the care their patients receive. CCPN allows North Carolina’s independent primary care clinicians to share best practices, improve quality of care  and participate in payer contracts which have been negotiated for the benefit of patients and primary care clinicians 

CCPN is the largest clinically-integrated network in North Carolina which has been created with the sole purpose to support NC’s independent primary care physicians. CCPN is uniquely positioned to provide independent clinicians the support they need, not just survive, but to THRIVE in the new healthcare environment.  CCPN welcomes Kintegra Health as a member of our CIN.
